Used seat belt anchor plate that has been cut out from a car in our salvage yard. Must be welded to floor pan. 6 total per car, 2 inboard in the transmission tunnel, and 4 on the rear seat pans. A very common breaking point on the upper portion of the driver side inner, can now be easily repaired. If you have a '68 or '69 Cougar or Mustang, check the driver seat back attachment for the recall / campaign fix. There should be a heavy bracket secured by a bolt over the seat back attachment.
